Online Self-Evaluation Survey for COVID-19

The survey is created by the Quebéc Ministry of Health and Social Services to provide information to the public about COVID-19.

This is not a medical assessment, but rather for people to self-assess before they call 1-877-644-4545. If you suspect you have COVID-19, KMHC recommends calling that number to speak with a nurse.
